Arizona Tax Extension

If you were approved for a federal tax extension, then Arizona will grant you an automatic six-month state tax extension. However, If you did not file a federal tax extension or you owe state income tax, then you must submit Arizona Tax Form 204.

Here are some mailing options:

If you are sending a tax payment with your extension request, mail Form 204 to:
Arizona Department of Revenue, PO Box 29085, Phoenix, AZ  85038-9085

If you are not sending a payment with your tax extension request, mail Form 204 to:
Arizona Department of Revenue, PO Box 52138, Phoenix, AZ  85072-2138
