How to Apply for an IRS Payment Plan in All 50 States

If you need to send in your form for an IRS payment plan, you have several options. Where to apply and send in your form depends on several factors.

If you owe under $25,000, then you can actually file online through the IRS. This is the quickest way and their online system does basically everything for you.

If you owe over $25,000 or just would rather use a physical form, then here are the addresses for you to send them to. If you do owe over $25,000, you need both the 9465 and 433-F IRS Forms.

Self Employed Individuals
If you are self employed and live in:

  • Illinois
  • Indiana
  • Iowa
  • Kansas
  • Kentucky
  • Michigan
  • Minnesota
  • Missouri
  • Nebraska
  • North Dakota
  • New Jersey
  • Pennsylvania
  • Ohio
  • South Dakota
  • West Virginia
  • Wisconsin

Then you should send your IRS payment plan forms and payments to the following address:

If you are self-employed and live in:

Wage Earner
Employed individuals send their tax payment plan forms to different addresses than self employed people. They do use the same forms, however.
